Open House: Autumn 2018 Schedule

Our Open Houses are always informal, and there is no fee or obligation when you stop by.

Once our practice is full, we will offer to potential patients the opportunity to participate in our Future Patient Campaign. We will begin receiving $250 non-refundable Reservation Fees per patient on a first-come-first-serve basis for future opportunity to participate in our innovative Direct Access Medicine® practice, a form of Direct Primary Care and Concierge Medicine that is guaranteed to make your healthcare experience both affordable and satisfying.

Autumn 2018 Open House Schedule: All Open Houses are held on the last Saturday of each month (excluding holiday weekends) from 9 AM to 11 AM at Summit Medicine and Pediatrics.

  • July 28th, 2018; Saturday morning from 9 AM to 11 AM
  • August 25th, 2018; Saturday morning from 9 AM to 11 AM
  • September 29th, 2018; Saturday morning from 9 AM to 11 AM

Summit Medicine and Pediatrics will CLOSE to acceptance of new patients at current terms and prices effective

WEDNESDAY, AUGUST 1, 2018!

Summit Medicine and Pediatrics has experienced outstanding growth and success in the last 4 years. As a result, we are almost at full capacity with current, established patients. We have the ability to accept five (5) more patients at current terms and prices if they engage with us prior to Wednesday, August 1, 2018, after which we will then initiate a Future New Patient Campaign in anticipation of maintaining optimal capacity.

What You Should Do Right Away

If you have family members, friends, neighbors or co-workers who have already expressed a desire to join our Direct Access Medicine® practice, but have not yet done so, we urge you to encourage them to come to our office right away to establish care. If they are among the next five (5) new patients to engage with us, the terms and prices of service will be honored.

For our current patients, it is very important to note that NOTHING changes with regard to pricing at your current service level; for the foreseeable future, our current patients will pay current pricing for all service levels. Effective August 1, 2018, the only change for current patients is that Service Level 1 will be available for those patients already at this service level, and will no longer be available to our established patients who are currently at Service Levels 2 through 4.

Future New Patient Campaign

Once we reach capacity with five (5) more new patients, or by August 1, 2018, we will change our terms and prices of service in the following way:

  • After August 1, 2018, Future New Patients who wish to join our practice will pay a $250 non-refundable Reservation Fee to be placed on our Future New Patient List. As established patients come to no longer require care from us, we will notify, in the order in which each Future New Patient has paid their Reservation Fee, our next new patient, assist the new patient in completing all engagement paperwork, and begin providing Direct Access Medicine® care for our new patient. It is important to understand that payment of a $250 Reservation Fee is non-refundable, and that engagement in healthcare services is neither implied nor expected until each Future New Patient is awarded their place in the practice. A place in our practice will be guaranteed to all Future New Patients within six (6) months of having paid their $250 non-refundable Reservation Fee.
  • After August 1, 2018, Future New Patients who wish to join our practice will have a choice among four (4) new service levels. Summit Medicine and Pediatrics new service levels are: A) Service Level 1 at $150 per patient per month ($450 per family of four per month); B) Service Level 2 at $300 per patient per month ($900 per family of four per month); C) Service Level 3 at $500 per patient per month ($1500 per family of four per month); and D) Service Level 4 (Hospice and Palliative Care Only) at $700 per patient per month. Future New Patients may change (one time per month) their service level to accommodate their changing healthcare needs. It is important to note that the structure and pricing of service levels will not change for our current, established patients for the foreseeable future, with the exception that Service Level 1 will no longer be available to our established patients who are currently at Service Levels 2 through 4.

The details of our new services levels are available for your review at your convenience.

All Future New Patients will be required to meet with Dr. Mike in consultation to discuss the principles of Direct Access Medicine® care and the terms and pricing of our service levels prior to being placed on our Future New Patient List. We do this to ensure that each Future New Patient has an opportunity to ask and have answered all questions prior to engaging with us.

We know that many of our current patients will have questions, and we are ready to assist. Please take a moment to call us at 480-981-9000 during routine office hours and we’ll answer all your questions. In addition, Dr. Mike will continue to host a monthly Open House from 9 AM to 11 AM on the last Saturday of each month (excluding holiday weekends).
